This document is issued by Trendlika Slime Academy, a workshop and education brand owned and operated by Trendlika, a sole proprietorship of Rathi Gunda, having its principal place of business at [Flat G-5, Shivam Apartment -1, Saptagiri Colony, Neredmet, Secunderabad,, Telangana 500094] GSTIN: 36AKIPG2779G1ZZ

By submitting the workshop registration form or otherwise consenting to participate in a Trendlika Slime Academy workshop, I, the parent or legal guardian of the participating child, acknowledge that I have read, understood, and agreed to the following terms in their entirety.

1. Participant Conduct & Safety Acknowledgment

I confirm and agree that:

• My child will follow all instructions, safety guidelines, and behavioural expectations communicated by the Trendlika instructor and workshop staff throughout the session.
• I understand that slime and all associated materials are strictly non-edible and must not, under any circumstances, be ingested, tasted, or placed in the mouth.
• Slime must be kept away from the eyes, mouth, nose, ears, and any open wounds, cuts, or areas of broken skin.
• Continued adult supervision is required when my child plays with the slime after the workshop, particularly in the presence of younger siblings, pets, or children below the age of three.
• Trendlika Slime Academy bears no responsibility for the misuse, alteration, or unsupervised use of materials after the conclusion of the workshop.
• I undertake to disclose, in advance and in writing, any allergies, skin sensitivities, medical conditions, or special requirements relevant to my child's participation.

2. Hygiene & Care Guidelines

I will ensure that my child and our household follow the recommended hygiene and product-care protocols, namely:

• Hands must be washed thoroughly with soap and water before and after engaging with slime.
• Slime must be stored in a clean, airtight container when not in use.
• Slime must not be placed on fabric, upholstery, hair, carpets, or other porous or absorbent surfaces.
• Slime must be kept away from direct sunlight, heat sources, and extreme temperatures to preserve its quality and safety.

3. Assumption of Risk & Liability Waiver

I acknowledge that, while Trendlika Slime Academy implements all reasonable safety measures, participation in any hands-on creative activity carries inherent risks. I voluntarily accept these risks on behalf of my child and agree as follows:

I hereby release, indemnify, and hold harmless Trendlika Slime Academy, its founders, employees, instructors, volunteers, and venue partners from any and all claims, damages, losses, or causes of action arising out of or in connection with my child's participation in the workshop, including but not limited to:

• Minor skin irritation, rash, or allergic reaction;
• Accidental misuse or mishandling of materials;
• Staining of clothing, accessories, or personal belongings;
• Any damages or incidents occurring outside the workshop environment or after the conclusion of the session.

4. Declaration of Authority

I hereby declare that I am the legal parent or guardian of the child named in the registration form, that the information provided is true and complete to the best of my knowledge, and that I am authorised to grant the consents and accept the terms set out above on the child's behalf.

5. Governing Law & Jurisdiction

This agreement shall be governed by and construed in accordance with the laws of India. Any dispute arising out of or in connection with these terms shall be subject to the exclusive jurisdiction of the competent courts at Hyderabad, Telangana.
